Yes, you need to download the new client information form, the HIPPA Privacy Statement form and a Release of Information form if you want to receive financial support for counseling from an ecclesiastical leader or if you would like a psychologist to consult with other family or medical personnel. If you want help checking benefits, please bring your insurance card.
We do not have a sliding scale. If you have no insurance or ecclesiastical support and have a crisis situation, a few scholarship slots for brief therapy may be available at a reduced cost. Please check for availability and for scholarship requirements.
If this is your first time participating in therapy, it can be a little scary just because you are doing something that you have never done before and most of us feel a little anxious when going into the unknown. In your first session, you will spend some time with your psychologist getting to know each other and learning a little more about the problem that has brought you to therapy.
At Mental Health the following fees apply: 60 min Couples Session $120; 50 min Individual Session $115; 50 min Bishop Pay $100; 50 min Group Session $40; 8 Week Hold Me Tight Class $400 per couple. Payment is due at time of service. We accept cash, credit or PayPal payments.
